Guidelines for Human Remains (HUM)

Requirements for Human Remains: 

  • Death Certificate 
  • Embalming Certificate (except for Muslim) 
  • Quarantine Permit (from Bureau of Quarantine – DOH Port Area MNL) 
  • Transfer Permit or Permit to Transport (from Municipality/City Health Office) 

Additional information: 

  • Four photocopies of each document. 
  • Original documents must be presented upon transfer of Cadaver. 
  • Coffin must be hermitically sealed and crated. 
  • Shipments must be at our Cargo Acceptance Office at least 5 hours before the flight. 
  • Cargo Estimate More or Less 20,000 PHP to 25,000 PHP. 

Requirements for Human Bones: 

  • Death Certificate 
  • Exhumation Permit from the City Health Office 
  • Quarantine Permit (from Bureau of Quarantine – DOH Port Area MNL) 
  • Transfer Permit or Permit to Transport (from Municipality/City Health Office) 

Human bones must be tendered in a container that will adequately protect the contents from damage with ordinary care and handling. 

 Requirements for Human Ashes: 

  • Death Certificate 
  • Cremation Certificate 
  • Quarantine Permit (from Bureau of Quarantine – DOH Port Area MNL) 
  • Transfer Permit or Permit to Transport (from Municipality/City Health Office) 

Ashes in urns must be shipped in a funeral urn or a similar sealed container and then placed inside a sturdy box or crate with inner cushion.
